Transportation Assistance
Assistance for the purchase or payment of transport (air/sea/land) or
expenses to enable the client to return to his home province or seek
further medical interventions in another place or to attend to emergency
concerns such as death, critical situations of family members or other
individuals in need. This includes those that require immediate
presence, such as but not limited to, attendance to a court hearing,
rescue of abused or trafficked relatives.
Criteria of Eligibility:
Must be a resident in the municipality
Must demonstrate an urgent need for transportation due to a crisis
situation medical condition, rescue of abused, court hearing, death,
and critical situations of family members.
Must belong to a low-income or financially distressed household
Specific Crisis Situations
Covered Applicants may qualify for transportation assistance if they
fall under any of the following categories:
a. Medical-Related Emergency
Individuals requiring immediate medical treatment in another location
due to the unavailability of necessary healthcare services in their
current area.
Patients requiring referral or transfer to a specialized medical
facility for further treatment.
Individuals needing medical evacuation (e.g., air or sea transport)
for life-threatening conditions.
b. Humanitarian and Social Welfare Needs
Individuals or families displaced due to natural disasters, armed
conflict, or other emergencies and needing transportation back to
their home province or safe location.
Survivors of domestic abuse, human trafficking, or other forms of
exploitation who need transportation for rescue, reintegration, or
legal proceedings.
Individuals traveling to attend court hearings related to legal
matters such as child custody, domestic abuse, or criminal cases
involving them or their dependents.
c. Family and Personal Emergencies
Individuals needing to travel due to the death of an immediate family
member (e.g., spouse, parent, child, or sibling).
Individuals who must urgently attend to a critical health situation of
a close family member in another location.
Individuals needing to assist in the rescue or recovery of a missing,
kidnapped, or trafficked relative.
Requirements

Updated Original Certificate of Indigency from Barangay with proof
of low income
- Medical Certificate referral (for medical transport request)

Death Certificate or Medical Report of a Relative (for family
emergency)

Police Report or legal documents (for cases of abuse, transferring
or court related cases).
